when trying to add third grenade puch, it just says go to equipment and get a pouch to unlock, i have grenade puches, but it still doesnt open the third slot wtf?

Third slot is from Large granade pouch, normal ones carry 2 granades
You need a large grenade pouch, not a normal grenade pouch.
Normal ones add 1 grenade, large ones add 2.
