I’m wondering does this perk 
work on Sniper Rifles?
Also how exactly does the “downed” works? What can put you in that state and what does not ? If I’m not mistaking when you get headshotted you cannot self revive correct?
So in theory if I have big backpack and put 4 medkits can I revive myself 4 times? is there a limit?

the following perk in the photo, yes, it does also work on snipers.
well, there are a couple of way to " down " someone, if they have a medkit in their backpack, they have more chances to get in a downed position and being able to ““revive”” their selves.
exactly, headshots, knives ( sometimes) axes, and explosions will most likely kill your opponent.
as long you don’t take fatal damage ( to the neck or head ) theorically, you can revive your self 4 times.
but to be honest, as i run my engy with 3 medkits on their back, they are usefull to be the medics of the squad.
